Leelanau Physical Therapy Clinic accepts most insurance plans. However, we encourage you to call the clinic in advance of your first appointment to verify your options and clarify your benefits. Please utilize the insurance benefit worksheet provided here when discussing and determining your benefits before your appointment.

Call your insurance company and complete the benefit worksheet to understand all requirements for consideration of reimbursement. Unfortunately, at this time we are unable to offer occupational therapy treatment to Medicaid beneficiaries.

What is Occupational Therapy?
